Eye Care Insurance Coverage
Attention TRICARE Patients
New dental and VISION coverage options are being offered to you for 2019; open enrollment to begin in November 2018.
At this time enrollment is optional. Any vision benefits would be in addition to your current Tricare health plan coverage. Active duty family members with Tricare Prime or Select get one routine eye exam per year (when over age 3). Retired Tricare Prime members get one routine eye exam every two years.
Be advised, Family Eye Care of the Carolinas (The Optical Shoppe) DOES NOT PARTICIPATE with any of the FEDVIP vision plan options. FECC does still participate with Tricare health plans and can provide routine eye care through them. We provide itemized receipts for you to file Out of Network for hardware.
It is suggested you consider your family’s eye care needs, preview the list of participating providers and compare plan costs to determine if enrollment is right for you.

Family Eye Care of the Carolinas Family Eye Care of the Carolinas in Aberdeen accepts and files a wide variety of insurance. We currently participate with the following VISION plans:
- Community Eye Care
We currently participate with the following MEDICAL plans:
- First Carolina Care
- NC Medicaid
- United Healthcare
Don’t see your plan on the list, call us or check your plans website or customer help number. If your policy requires an authorization you will need to obtain one prior to your visit. We welcome your questions on insurance coverage and financial options. We’re here to help!
If you have separate vision and medical insurance, make sure you inform staff which one you will be using when making your appointment. If your insurance requires authorizations, make sure to obtain the appropriate one at least 7 days prior to appointment. Services provided without authorization are the full responsibility of the patient.
You should wear/bring all eyeglasses and contact lens to your visit as well as a list of all prescription and over the counter medications you currently take. All comprehensive eye exams dilation. If you are uncomfortable driving after having your eyes dilated, bring a driver with you.
Please be aware that all co-payments and deductibles are due on the day of service. A $20 statement fee will be applied to any account where co-payment, deductible, and/or non-covered services charge is not paid in full at check out on the day of service.
Patients are encouraged to arrive promptly to all scheduled appointments. We understand that you may need to cancel or reschedule your appointment. If you need to cancel an appointment, please call at least 24 hours in advance to reschedule. Appointments that are cancelled with less than 24 hours notice are a “no show,” and are subject to a $40 cancellation fee.
For your convenience, we also offer various alternate payment methods for vision services rendered:

You must REVIEW your entries and then click SUBMIT in order for them to be saved. Keep your confirmation number for reference to any problems.
Established patients should complete registration information every year to ensure we have your most current contact and insurance information as well as any health changes you have experienced. If you have not been seen in our office since September 2015, you are advised to submit your information.

HIPAA Policy of Family Eye Care of the Carolinas
Print and complete the following forms as needed:
Financial Policy and HIPPA Policy – when financial responsibility changes or additional people need to be added to HIPPA permissions
Adult Strabismus and Diplopia Referral Form – must be completed for any adult patient to be seen in our specialty clinic for eye muscle/alignment issues or double vision
Release of Medical Records – to send or request transfer of medical records
Refraction Waiver – if your medical policy does not cover refractions (FECC does not participate with vision plans)
